Additional & transitional care
Putting patients first and preventing hospital readmissions
We provide regular monthly NP visits for ongoing primary and chronic care. Additional visits are scheduled when medically necessary to best support our patients. For those recently discharged from the hospital or rehab, our Transitional Care program offers extra support during the critical recovery period before returning to our standard monthly visit schedule.
