A full care team behind every visit

1. Nurse Practitioners (NP)

   Our NPs lead every patient's care, carrying out regular monthly home visits — routine check-ups, sick visits, chronic condition management, medication reviews, and vaccinations — plus additional visits when medically necessary.

2. Care Coordinators (CCM)

   As part of our Chronic Care Management program, a Care Coordinator performs a monthly wellness check between NP visits — seeing how you're feeling and helping with refills, referrals, forms, and any other needs.

3. Triage & Medical Concern Team

   For non-emergency medical concerns, call us first. Our triage team assesses the situation, answers questions, and schedules an additional NP visit when medically necessary — often saving an unnecessary ER trip.

4. Medical Forms Department

   We assist with any medical forms that need to be completed by a primary care provider — M11Q and more — so the paperwork never falls on you.

5. Scheduling & Intake

   Our scheduling team is your first phone call — verifying insurance, booking visits within 48 hours, and coordinating our Transition of Care program for patients recently discharged from the hospital or rehab.
