Meir Vachss, DNP, FNP-BC
Founder & CEO
Dr. Meir Vachss, DNP, FNP-BC, is the Founder and CEO of Kings County House Calls. With more than 20 years of hands-on clinical experience in medicine and a Doctor of Nursing Practice degree, he remains an actively practicing board-certified Family Nurse Practitioner who continues to see patients in the field while leading the organization. Driven by a clear vision to serve an often-overlooked population, Dr. Vachss built the practice to deliver consistent, high-quality medical care directly to homebound geriatric patients. Recognizing that many of these individuals go months between traditional office visits, he created a proactive, home-centered model that brings comprehensive resources to the patient — closing care gaps, preventing complications, and significantly reducing unnecessary hospitalizations. Under his leadership, Kings County House Calls has grown into a scalable, multidisciplinary organization committed to exceptional outcomes for complex, homebound patients across Brooklyn and the New York area. Dr. Vachss remains deeply involved in clinical care, ensuring the practice never strays from its core mission: outstanding, compassionate medicine where patients need it most.

Avraham Y. Vachss, J.D.
Chief Operating Officer
Avraham Y. Vachss, J.D., is a graduate of Cardozo School of Law and serves as Chief Operating Officer for Kings County House Calls and its affiliated companies. With a strong foundation in law and a deep commitment to operational excellence, Avraham oversees internal operations across all departments, ensuring efficiency, compliance, and seamless coordination in the delivery of high-quality home-based medical care. He plays a pivotal role in maintaining and expanding strategic business relationships, actively engaging with referral sources, vendors, partners, and stakeholders at the highest levels. His proactive approach to relationship-building and business development has been instrumental in supporting the practice’s continued growth and success in serving the complex needs of homebound patients.

Amyanerys (Amy) Lopez, MSN, FNP-BC, a Family Nurse Practitioner with 1.5 years of NP experience and more than 15 years in healthcare. Amy has a strong clinical background that includes approximately 8 years as a registered nurse, with extensive experience in emergency department care, home care, and community clinic settings. Passionate about providing compassionate, patient-centered care and supporting patients through both urgent and ongoing health needs.

Dr. Andrew Wolf, DNP, FNP-C, RN earned his Doctor of Nursing Practice (DNP) from Pace University and his nursing degree from Long Island University. He is dedicated to delivering compassionate, patient-centered care, with a focus on improving health outcomes, promoting independence, and enhancing the quality of life for every patient he serves.

Ariel Steinherz

Dr. Ariel Steinherz is a board-certified Adult-Gerontology Primary Care Nurse Practitioner dedicated to providing compassionate, patient-centered medical care in the comfort of patients’ homes. He earned his Doctor of Nursing Practice (DNP) degree from Gannon University and has experience caring for older adults and medically complex patients with chronic conditions, acute illnesses, and mobility limitations. His clinical interests include chronic disease management, preventive care, wound care, and improving access to high-quality healthcare for homebound individuals. Dr. Steinherz is passionate about building meaningful relationships with patients and their families while delivering personalized, evidence-based care that promotes independence, comfort, and quality of life.

Asher Kohanteb, MSN, FNP-BC, is a Family Nurse Practitioner with Kings County House Calls, providing comprehensive, patient-centered primary care to geriatric patients in the comfort of their homes. Drawing on six years of combined experience as both a Registered Nurse and Nurse Practitioner, including practice in emergency medicine, a Level I trauma center, and pediatric emergency care, he brings a broad clinical foundation to every patient encounter. Asher is committed to delivering compassionate, accessible, and preventative care that is thoughtfully tailored to each individual's unique needs, combining modern, evidence-based medicine with a personal touch. Jenny Krinets
MSN, FNP-C
Jenny Krinets, MSN, FNP-C, began her healthcare career with clinical training in Traditional Chinese Medicine (TCM), 4 years in Reidman college Israel, completing a 480-hour internship at Zhejiang Provincial Hospital of Traditional Chinese Medicine in Hangzhou, China. Training included acupuncture, Tuina, Herbs. Later earned an Associate Degree in Nursing from Kingsborough Community College, a Bachelor of Science in Nursing from New York City College of Technology (City Tech), and a Master of Science in Nursing–Family Nurse Practitioner from United States University. With six years of experience as a home care registered nurse and now as a board-certified Family Nurse Practitioner. Education included primary care, preventive medicine, chronic disease management, and integrating evidence-based practice with holistic principles to improve patient outcomes.

Julia Ilyayeva MSN, FNP - BC earned a Bachelor of Science in Nursing from Adelphi University and worked as a Registered Nurse at NYU Langone Health for five years. Julia cared for patients with advanced heart failure and heart transplants in Cardiology and later transitioned to the Cardiac Critical Care Unit (CCU) during the COVID-19 pandemic while completing her Master of Science in Nursing at Hunter College as an Adult-Gerontology Nurse Practitioner. After graduation, Julia briefly practiced in Obstetrics and Gynecology before transitioning to home-based primary care, where she has been providing comprehensive, compassionate, patient-centered care.

Yisroel Rosenfeld, MSN, FNP-BC
Yisroel Rosenfeld began his career in healthcare as an EMT at Senior Care EMS. Seeing firsthand how patients frequently fell through the cracks of the healthcare system inspired him to pursue advanced clinical training to make a deeper impact. He earned a Bachelor's degree in Biology from Touro University, followed by a Bachelor's degree in Nursing from New Jersey City University. After gaining valuable clinical experience working at Maimonides Medical Center, Yisroel advanced his education by completing his Family Nurse Practitioner program at United States University. During his advanced training, he gained extensive hands-on experience under the mentorship of Joseph Kaplovitz, DO, Abraham Eisenman, FNP-BC, and Barry Gordon, MD.
