Managing Medications Safely at Home: Tips from Our NPs

Taking multiple medications is common for many of our homebound patients. When managed well, medications improve quality of life. When not, they can cause confusion, side effects, or even trips to the emergency room. Here are practical tips from our Nurse Practitioners.
1. Keep an updated medication list
Always maintain a current list that includes:
- Medication name
- Dosage and frequency
- Reason for taking it
- Prescribing doctor
Bring this list (or the actual bottles) to every NP visit. We review it together at every appointment.
2. Use a simple organization system
- Pill organizers (weekly or daily)
- Medication reminder apps on your phone
- A dedicated "medication station" in an easy-to-reach location
3. Request refills proactively
The best time to request a refill is during your NP visit. If you run low between visits, call the office or your CCM coordinator right away. We work quickly to prevent gaps in treatment.
4. Watch for side effects or interactions
Tell your NP about any new symptoms — even if you think they're minor. Dizziness, stomach upset, confusion, or changes in energy level can sometimes be related to medications.
5. Involve your care team
Our Nurse Practitioners, CCM coordinators, and pharmacy partners work together. Never hesitate to ask questions — we'd rather explain something twice than have you unsure about what you're taking.
