Managing conditions like high blood pressure or heart failure can feel overwhelming — especially when you're homebound. Our Remote Patient Monitoring (RPM) program gives you and your care team better visibility into your health between visits.
How RPM works
If you qualify, we send you a simple, easy-to-use blood pressure monitor at no cost. You take readings at home (usually once or twice a day) and the data is automatically uploaded for our clinical team to review.
Our Nurse Practitioners and care team monitor the trends. If something needs attention, we reach out proactively — often before you even notice a change. This allows us to adjust medications quickly and avoid unnecessary emergency visits.

Who is eligible?
